Job summary

Enhabit Home Health & Hospice in Mobile, AL is seeking a licensed healthcare professional to perform admission visits and deliver care in the patient’s home under physician orders and the established plan of care.

You will build trusting relationships with patients and families, coordinate with the care team, and promote safety, quality, and compassionate in-home services across our network.

Qualifications

• Must be a graduate of an approved school of clinical education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience as a licensed professional.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Perform admission visits and establish the relationship between patient, family, and company.
• Deliver services per physician orders and the plan of care (POC).
