Job summary
BAYADA Home Health Care is seeking a Registered Nurse (RN) Admissions for home health visits in Harrington, Delaware. This full-time role involves performing admissions nursing duties within a multi-disciplinary team to provide rehabilitative care and skilled nursing at clients' homes.
The ideal candidate holds a DE RN license, has at least one year of clinical experience, and can travel to cases. EMR experience and strong time-management are preferred, with commitment to BAYADA values and 1:1
Qualifications
- A current DE RN license.
- Minimum of one year of clinical experience.
- Home health experience preferred.
- Graduation from an approved nursing program.
- Ability to work independently and manage time.
- Strong interpersonal skills and computer literacy.
- Ability to travel to cases as assigned.
Responsibilities
- Follow designated care plans for patients in home settings.
- Perform home visits to clients in designated geographic territories.
- Administer medications, wound care, treatments, and procedures.
- Monitor patient conditions and report changes to supervisors.
- Document doctors' orders and assessments accurately.
- Coordinate case management and care delivery.
