Avamere Transitional Care of Puget Sound in Tacoma, WA is seeking an RN Admissions Nurse to provide comprehensive patient care, assess residents, administer medications, and coordinate care with the interdisciplinary team. The role includes supervising LPNs/CNAs and ensuring accurate documentation of resident care.

Requirements include a nursing degree, current RN license in Washington, CPR/BLS, and EMR documentation experience.

Qualifications

• Must possess a nursing degree from an accredited college or university.
• Must possess a current, unencumbered RN license in the state; BSN preferred.
• Must maintain active CPR/BLS during employment.
• Experience in long-term care or geriatric rehab preferred.
• Experience with electronic medical records (EMR) documentation.

Responsibilities

• Patient assessment and care planning with the interdisciplinary team.
• Administer medications and monitor responses as prescribed.
• Document vital signs and health changes accurately.
• Coordinate care and communicate with physicians, therapists, and staff.
• Supervise and train LPNs and CNAs as assigned.
• Maintain thorough care documentation and participate in QA initiatives.
