Job summary

VITAS Healthcare in Wisconsin is seeking a Hospice Admissions Nurse to bridge hospital or community care with comfort-focused hospice services.

The position requires a Registered Nurse with a valid state license and a minimum of two years nursing experience (hospice, palliative, ER, ICU, home health or case management preferred). You will assess patients, educate families, coordinate admissions, and support end-of-life care decisions.
