VITAS Healthcare seeks an experienced Hospice Admissions Nurse to coordinate patient admissions during home visits. You will assess eligibility, educate families about hospice services, and work with physicians and care teams to ensure smooth transitions.
Applicants should have an active RN license in Illinois and at least 2 years of nursing experience, preferably in hospice or related fields. This role includes mileage reimbursement and full benefits.
Salary Range: $42- $46/Hour, plus mileage reimbursement and full benefits
Schedule: 5 days a week, 11am to 8pm, rotating weekends, performing home visits.
