Job summary

Addus HomeCare in Albuquerque, NM seeks an Admissions Registered Nurse for a full-time daytime schedule. You will collaborate with the interdisciplinary team to assess patients and coordinate care, ensuring timely visits and accurate charting.

Requirements include RN licensure, current CPR, and at least 1 year in Home Health. A valid driver’s license and auto insurance are required. Great benefits and tuition reimbursement are part of the package.

Qualifications

• Graduate from an accredited registered nurse program.
• Licensed as a registered nurse in the state of practice.
• Maintains a current CPR certification.
• 1 year Home Health experience required.
• Valid driver’s license and proof of insurance is required.

Responsibilities

• Obtain data and assess patient information to determine needs and coordinate care with the interdisciplinary team.
• Initiate communication with the attending physician and other agencies to coordinate optimal care.
• Provide regular visits to reassess patient needs and update the care plan.
• Perform nursing procedures as directed by the physician.
• Maintain accurate patient records and care plans.
• Supervise and document delivery of care by Home Health Aide as required by guidelines.

Full-Time

Schedule: Friday through Tuesday 8am to 5pm

What You�ll Do:

• Obtain data and assess necessary information from the patient physical, psychological, social, and spiritual factors that may impact the patient and family's needs and initiate intervention by other members of the interdisciplinary team
• Initiate communication with the attending physician, interdisciplinary team, and other agencies for the purpose of coordinating optimal care
• Provide regular visits to re-assess the patient's needs and update the plan of care
• Perform therapeutic, preventative, and rehabilitative nursing procedures as directed by the attending physician
• Perform clinical pronouncement of death and provide support to the family
• Maintain accurate patient records, charts, progress notes, and reports and prepare care plans timely
• Supervise and document delivery of care by the Home Health Aide as required by Federal, State, and Company guidelines

Qualifications:
