Job summary

Glens Falls Center seeks an Admission Registered Nurse to coordinate and complete admissions for residents in our skilled nursing and rehab facility. The RN will perform comprehensive nursing assessments, develop initial care plans, reconcile medications, and ensure safe, person-centered transitions from hospitals and the community.

This role requires compassionate patient-centered care and meticulous medication reconciliation.

Responsibilities

• Coordinate and complete admissions for residents.
