Job summary

Glens Falls Center for Rehabilitation and Nursing is seeking an experienced RN Admissions Nurse to manage admissions and initial care planning for residents in Glens Falls, NY. The role involves coordinating with the interdepartmental team, reviewing care plans, and ensuring compliant documentation.

Responsibilities include developing and directing the admissions process, auditing records for standards, participating in discharge planning, and maintaining HIPAA confidentiality.

Qualifications

• Active NYS Registered Nurse license required.
• Prior admissions experience in long-term care settings preferred.
• Strong clinical assessment and decision-making skills required.
• Ability to lead and supervise staff.
• Excellent communication and interpersonal abilities.
• Computer proficiency in MS Office (Excel, Word, Outlook).

Responsibilities

• Initiate admission assessments for new residents and hospital returns and initiate initial care plans.
• Assist in development and direction of the Nursing Department as directed by the DON.

• Audit records for facility standards compliance.
• Participate in discharge planning when needed.
• Review and adjust residents’ care plans as conditions change.
• Complete charting, MDS, care plans and related documents per policy.
• Maintain HIPAA confidentiality and corporate guidelines.
• May supervise or direct resident care as assigned.

Glens Falls Center is hiring a RN Admissions Nurse in Glens Falls, NY.

Duties Include:

• Initiate admission assessments on new Residents / hospital returns & initiates initial care plan.
• Assist with the development & direction of the Nursing Department as directed by the DON.
• Review PRI's & coordinate all components of the admission process with the interdisciplinary team.
• Responsible for auditing records to ascertain compliance with facility standards.
• Will participate in discharge planning when necessary.
• Review & evaluate Residents’ care plans when conditions warrant a change as necessary.
• Perform required charting according to policy / procedure.
• Complete MDS, care plans and other documents as assigned.
• Maintain the confidentially of all Resident information as per HIPAA and Corporate guidelines.
• May be assigned to supervise or direct Resident care as necessary.

Requirements/Qualifications:

• Must have an active / unrestricted NYS Registered Nurse License
• Prior experience as an admissions nurse in LTC settings
• Should have excellent clinical assessment skills
• Ability to make good independent decisions
• Strong leadership and supervisory skills
• Computer savvy; proficient in MS Office (Excel, Word, Outlook)
• Long-Term Care Experience Preferred
• Excellent communication and interpersonal skills
