Job summary

St. Joseph Hospice in Shreveport, LA is seeking a Hospice Admissions RN to assess patients, coordinate care, and educate families. Schedule: 11am-7pm; travel to patient homes or facilities as needed.

You will work with physicians and the hospice team to implement the Plan of Care, maintain records, and support on-call duties. Louisiana RN license and CPR are required; hospice experience is a plus; ESOP and benefits are provided.

Qualifications

• Louisiana RN license is required.
• Current CPR certification is required.
• Experience in hospice care preferred.
• Ability to visit patients at home or facilities.
• Proficient with EMR systems.

Responsibilities

• Perform initial and ongoing patient assessments.
• Assist in developing and implementing the Hospice Plan of Care.
• Educate patient and family on Plan of Care, including medications.
• Act as primary liaison between physician, patient/family and hospice team.
• Maintain accurate and updated medical records.
• Participate in agency on-call schedule.
• Abide by policies, procedures, and compliance guidelines.

Job Duties:

• Performs the initial and on-going assessment of the patient
• Assist in developing and implementing the Hospice Plan of Care
• Educate patient and family on Plan of Care, including medication administration, dressing changes, oxygen administration, Hospice philosophy and care of the terminally ill patient
• Act as primary liaison between physician, patient/family and hospice team
• Maintain an accurate and updated medical record
• Makes routine supervisory visits
• Participates in agency on-call schedule
• Abide by company policies and procedures, compliance guidelines, applicable laws, and regulations promptly reporting any suspected violation of compliance standards

Qualifications:

• Registered Nurse (RN) license required in State of Louisiana
• Current CPR certification required
• Make patient visits at their place of residence or facility
• Call will be required once a week as needed
• Knowledge of EMR is required
• Previous Hospice experience needed
