Job summary

Moments Hospice in Wisconsin is seeking an Admissions RN to join our team on a full-time basis. This role focuses on admissions coordination, patient assessment, and ensuring a smooth transition into hospice care.

Hours are Monday-Friday 12:00 pm-8:00 pm. A valid RN license and 1–2 years of nursing experience are preferred. A company car with fuel and insurance is provided at a minimal cost to the employee.

Qualifications

• Valid RN license in the state where duties are performed.
• 1–2 years of nursing experience preferred.
• Driver's license, insurance and a vehicle may be required.
• Utmost compassion and integrity in patient care.
• Strong presentation skills.
• Previous hospital, physician practice, LTC or ALF experience.

Responsibilities

• Participate in information gathering, analysis, education and form completion of admissions.
• Evaluate patients for hospice appropriateness through medical records review.
• Obtain agreement for admission of referred patients and families.
• Analyze data to facilitate admission orders.
• Build referral source relationships to support service quality.
• Maintain clinical and professional competence.
• Develop effective internal relationships with admissions and patient care staff.
• Educate patients and families about hospice and palliative care.

Nurses looking for autonomy and variety should consider practicing in Hospice Care. As an Admissions RN, you will join a team that is dedicated to making our patients' final days, weeks, and months as comfortable as possible.

Hours for this position will be Monday-Friday 12:00 pm-8:00 pm

Responsibilities:

• Participation in the information gathering, analysis, education and form completion aspects of the admissions process
• Evaluate patients for hospice appropriateness, e. g. medical records review as requested.
• Obtain agreement for admission of referred patients and families who meet admission criteria.
• Analyze data collected to facilitate/obtain admission orders
• Build effective referral source relationships to support the high quality and responsiveness expectations associated with Moments Hospice
• Maintain clinical and professional competence.
• Take the initiative in developing effective relationships internally with the appropriate admissions and patient care resource persons.
• Utilize interpersonal skills to help appropriately educate and address any questions the patient and/or patient’s family has concerning hospice and palliative care

Requirements:

• Valid Registered Nurse (RN) license
• Driver's license, insurance and vehicle
• 1-2 years of nursing experience (preferred)
• Utmost compassion and integrity
• Currently licensed as a R.N. in the state where duties are performed.
• Previous hospital, physician practice, LTC or ALF experience
• Strong Presentation Skills
• Previous sales or service experience preferred
