Job summary

The Actors Fund Home in Englewood, NJ seeks a full-time Nursing Home Admissions Coordinator to manage intake and admissions for new residents. The role focuses on building strong referral networks with hospitals and discharge planners across New Jersey and New York to maximize occupancy and census.

You will coordinate timely admissions, assess referrals, understand Medicare and insurance benefits, complete paperwork including Medicaid, and work with the care team to ensure smooth transitions of

Qualifications

• Bachelor's degree preferred in administration, communications or related field.
• Excellent verbal and written communication skills.
• Proficient in math and MS Office Suite (Excel, Word, Outlook, PowerPoint).
• Experience in a nursing home/assisted living or acute healthcare setting strongly preferred.

Responsibilities

• Develop, maintain, and expand referral relationships with hospitals and healthcare providers across NJ and NY to support admissions.
• Coordinate admissions, evaluate referrals, and ensure timely admissions.
• Complete admission paperwork including Medicaid applications and ensure eligibility.
• Maintain electronic charting and admissions documentation.
• Provide tours and information to applicants and families; ensure proper documentation.
