Job summary

West Delray Nursing and Rehabilitation Center in Palm Beach, FL is seeking an experienced Admissions Director to lead the admissions team and drive census growth. You will manage the admissions process, build referral relationships, and ensure smooth transitions for patients and families.

The role requires healthcare admissions experience, strong knowledge of Medicare/Medicaid and insurance processes, and the ability to collaborate with interdisciplinary teams to achieve occupancy and revenue

Qualifications

• Minimum of 2 years of healthcare admissions, intake, case management, or business development experience.
• Previous SNF or post-acute care experience strongly preferred.
• Previous Admissions Director or Admissions Coordinator experience required.
• Strong knowledge of Medicare, Medicaid, Managed Care, and insurance authorization processes.
• Understanding of healthcare regulations related to admissions, transfers, and discharge planning.
• Experience working with interdisciplinary clinical teams.
• Proficiency with PointClickCare, CarePort, NaviHealth, AllScripts, or similar referral platforms.
• Excellent communication, organizational, customer service, and relationship-building skills.

Responsibilities

• Oversee the admissions process from referral through admission.
• Build and maintain strong relationships with hospitals, physicians, case managers, and referral sources.
• Review clinical documentation to determine admission appropriateness and authorization requirements.
• Coordinate with nursing, therapy, social services, and finance to ensure seamless patient transitions.
• Monitor bed availability, admissions activity, and census development.
• Educate referral partners on facility services, specialty programs, and clinical outcomes.
• Support patients and families throughout the admission process.
• Partner with facility leadership to achieve occupancy and revenue goals.
• Ensure compliance with federal, state, and facility admission regulations.
