Job summary

Avalon Care Center in Bellingham is seeking an Admissions Nurse to join our team. This full-time role offers a wage range of $49.50–$54.05 per hour and a Tue–Sat schedule from 12:00 pm to 8:30 pm.

Applicants should be proficient problem solvers with strong communication and organizational skills. Placement within the wage range is determined by experience, qualifications, and ability to perform essential functions.

Qualifications

• 1–3 years clinical nursing experience, with at least two years in long-term care and two years in administration/consultation/survey involvement.
• Strong problem-solving, communication, and organizational skills.

Responsibilities

• Complete admission assessments for incoming residents including physical and clinical evaluations.
• Coordinate admissions to ensure smooth transitions into the facility.
• Review hospital records, physician orders, medications, and treatment plans prior to admission.
• Collaborate with physicians and the interdisciplinary team to support continuity of care.
• Ensure accurate and timely completion of admission documentation, authorizations, and medical records.
• Educate residents and families about admission procedures, care plans, and facility policies.
• Assess residents for level of care and identify immediate clinical or safety concerns.
• Coordinate room assignments, equipment, and resident care needs at admission.
• Maintain compliance with state, federal, facility, and HIPAA regulations.
• Assist with resident transfers and discharge coordination as needed.
