Job summary

Avalon Care Center Bellingham is seeking Admissions Nurses to join our team in Washington. This full-time role offers $43.50-$48.05 per hour and a Tue-Sun schedule from 12:00 pm to 8:30 pm.

Responsibilities include completing admission assessments, coordinating resident admissions, reviewing records, and educating residents and families on procedures, care plans and facility policies while ensuring HIPAA compliance.

Qualifications

• 1–3 years of clinical nursing experience with LTC exposure preferred.
• Experience in administration, consultation, or survey involvement.
• LPN/Licensed practical nurse license in good standing.

Responsibilities

• Complete admission assessments for incoming residents, including physical and clinical evaluations.
• Coordinate admissions to ensure a smooth transition into the facility.
• Review hospital records, physician orders, medications, and treatment plans prior to admission.
• Collaborate with physicians and interdisciplinary team to support continuity of care.
• Ensure accurate and timely completion of admission documentation, authorizations, and medical records.
• Educate residents and families regarding admission procedures, care plans, and facility policies.
• Assess residents for appropriate level of care and identify immediate clinical or safety concerns.
• Coordinate room assignments, equipment, and resident care needs upon admission.
• Maintain compliance with state, federal, facility, and HIPAA regulations.
• Assist with resident transfers and discharge coordination as needed.
