The Admissions Registered Nurse (RN) provides hospice and home care services, conducts initial assessments, initiates plans of care, and collaborates with the interdisciplinary team to meet patients’ and families’ goals.
Qualifications
- Current RN license in the state where the employee will be working.
- Minimum of one (1) year of nursing experience; hospice or hospital experience preferred.
- Experience working with an EMR/EHR system.
- Strong clinical assessment skills with a preferred background in the emergency department or ICU.
- Ability to effectively communicate in English (verbal and written); bilingual in Spanish preferred.
- Ability to present services in a professional and articulate manner.
- Valid driver’s license and automobile insurance per company policy; reliable transportation to meet schedule.
- Ability to work in a team environment and intervene appropriately.
- Ability to manage multiple priorities simultaneously and handle the emotional stress of the workload.
- Ability to perform essential functions and physical requirements (lifting patients/equipment, bending, pushing/pulling, kneeling) with or without reasonable accommodation.
- Strong goal/outcome orientation with exceptional time‑management skills.
- Excellent customer service focus.
- Active BLS certification for healthcare professionals from the American Heart Association or Red Cross.
Responsibilities
- Complete clinical admissions for all company services including explaining services, addressing patient/family concerns, and providing empathetic care.
- Identify admission objections, concerns, or other entry barriers and provide appropriate responses.
- Educate patients/families on admission requirements and address immediate needs.
- Complete informed consent and initial assessment according to regulations and company policy.
- Provide information to physicians and initiate a plan of care to address the patient’s immediate needs.
- Initiate skilled nursing interventions to prevent complications, alleviate symptoms, and maximize physical and emotional comfort.
- Obtain physician orders and complete documentation per company policy.
- Act as the company representative at assigned facilities, facilitate referrals to all service lines, and work closely with referring hospitals, physicians, and the public.
- Communicate frequently with Care Navigation staff to ensure timely follow‑up; inform the Care Navigation team when assignments are complete.
- Provide all necessary clinical communication using SBAR and discuss any potential needs with after‑hours staff.
- Obtain consents for services and arrange safe discharge in an efficient manner for referred and eligible patients while meeting productivity expectations.
- Obtain medical record information for referrals.
- Develop strong relationships with case managers, physicians, and other stakeholders at facilities.
- Work with Professional Relations Representatives (PRRs) to provide education on all available services.
- Provide care for GIP patients in a hospital, including daily visits, professional management, documentation, and efficient discharge.
- During emergencies, be prepared to report to a location designated by the company ahead of scheduled date/time; stay overnight(s) as required.
- Perform other duties as assigned.

Compensation
Pay range: $72,595.84 – $108,893.76.
