Job summary

Pine Forest is seeking an Admissions Nurse RN in Jackson, Mississippi to evaluate and admit patients to our services in accordance with policies and standards. You will conduct comprehensive assessments, determine eligibility, obtain consents, and initiate individualized care plans for safe transitions into care.

The role requires 3–5 years of nursing experience (preferably in long-term care) with admissions experience considered an asset.

Qualifications

  • Associates or Bachelor's degree in nursing is required.
  • 3–5 years of nursing experience, preferably in long-term care; admissions experience is a plus.

Responsibilities

  • Reviews medications on referrals and analyzes availability of medications and clinical capabilities.
  • Transcribes physician’s orders and queries the physician as needed to ensure accuracy of resident medical information prior to admitting the resident.
  • Reviews and evaluates resident diagnoses and overall condition related to criteria for skilled coverage and medical eligibility for long-term care placement before admitting the resident.
  • Assists with processing referrals timely on a daily basis as assigned by the Admissions Director.
  • Identifies and documents primary and other diagnoses codes for all admissions and readmissions in the medical record based on available information.
  • Informs the Admissions Director of obstacles to admitting residents and recommends solutions.
  • Communicates with interdisciplinary team members regarding clinical needs such as equipment, medications for pending admissions.
  • Speaks with physicians to clarify medical information when necessary.
  • Participates in offsite referral patient assessments as needed.
  • Facilitates completion of all required clinical assessments for New Admits.
  • Performs skin audits, order audits, referral reviews.
  • Serves as the Admissions Coordinator backup and interacts with referral sources.
  • Develops long-range and short-range marketing plans and goals aligned with facility budget.
  • Develops and implements promotional activities highlighting quality care and services.
  • Tracks marketing data and prepares reports for Administrator.
  • Lectures before groups on health and long-term care services.
  • Acts as liaison with physician offices, discharge planners and other referral sources.
  • Maintains good rapport with social services and nursing departments; maintains professional confidentiality.

JOB SUMMARY: M-F 8am-5pm $45,000 - $54,000 Annually

The primary purpose of the Admissions Nurse RN is to evaluate and admit patients to the organization’s services in accordance with established policies, regulatory requirements, and professional standards of nursing practice. This Nurse conducts comprehensive assessments, determines patient eligibility for services, explains care options, obtains required consents, and initiates individualized care plans that promote safe and effective transitions into care. It also serves as the first clinical contact for new patients and families, ensuring that admissions are completed accurately, efficiently, and with compassion. This position requires an in-depth understanding of clinical criteria, regulatory compliance, and the ability to educate patients and caregivers about available services and expectations of care.

JOB DUTIES & RESPONSIBILITIES:
Administrative Functions

Communicates with interdisciplinary team members timely regarding clinical needs such as equipment, medications, etc. for pending admissions.

Speaks effectively with physicians when necessary to clarify medical information

Participates in offsite referral patient assessments as necessary.

Facilitates completion of all required clinical assessments for New Admits.

Performs skin audits, order audits, referral reviews.

Serves as the Admissions Coordinator backup and interacts with referral sources.

Develop long-range and short-range marketing plans, establishing goals and objectives consistent with the overall goals and annual budget of the facility.

Develop and implement promotional activities that highlight the facility’s ability to provide quality care and services.

Track relevant marketing data such as inquiries by number, quality and source, conversion ratios, admission patterns, census and mix, and discharges and prepare reports as required by the Administrator.

Lecture before groups relative to health and long-term care services.

Act as a liaison with physician offices, discharge planners and other referral sources.

Develop and maintain a good working rapport with other departments within the facility, specifically social services and nursing.

Maintain professional confidentiality.

JOB REQUIREMENTS:
Education

Associates or Bachelors degree in nursing required.

Experience

Must have three to five (3-5) years’ experience in nursing, preferably in long term care. Admissions experience also preferred.
