Job summary

Diversicare is seeking an Admissions Coordinator to manage all aspects of patient and resident admissions, coordinating with the care team for safe, seamless transitions and strong referral relationships. You will collaborate with physicians, referral sources and the care team to meet admission targets, demonstrate center features, and uphold the Diversicare Service Standards.

This full-time role requires solid office experience, multitasking ability and a customer-focused approach to deliver

Qualifications

• Office experience with basic computer use and processes.
• High school diploma required.
• 1–2 years’ experience in skilled or long-term care preferred.
• Strong multi-tasking, communication skills and HIPAA compliance.

Responsibilities

• Coordinate all aspects of patient and resident admissions to the center for a safe, seamless transition.
• Develop relationships within key referral sources, physicians and healthcare professionals.
• Collaborate with team members to meet admission and key account management goals.
• Accurately represent center’s features, benefits and impact; conduct tours per standard procedures.
• Champion quality of care and advocate for the customer experience.

• Background in office work, including use of equipment, computer, and processes
• High school diploma
• 1 to 2 years’ experience in skilled care or long term care preferred
• Strong multi-tasking skills, communication skills, and HIPAA compliance track record
