Bridge Care & Community Connections: Admissions Liaison
Are you a relationship-builder who loves navigating complex puzzles to help people? We are looking for a dynamic Admissions Liaison to serve as the face of our community.
In this role, you aren't just filling beds—you are guiding families, hospital discharge planners, and seniors through one of life's most challenging transitions. You will blend the heart of healthcare with the strategy of sales and the technical knowledge of insurance navigation (MaineCare, Medicare, and commercial insurance) to ensure a seamless entry into our care teams while upholding our unwavering commitment to residents' rights, safety, and dignity.
Key Responsibilities & Essential Functions
- Drive Census Growth & Relationships: Build and maintain excellent relationships with referral sources, hospitals, physicians, discharge planners, and community organizations. Make regular visits to area hospitals and represent the facility at vendor fairs, expos, and marketing/sales functions.
- Be the First Guide & Clinical Reviewer: Respond rapidly to referral inquiries, conduct clinical reviews and financial pre-screenings of potential admissions, and contact patients and families to prepare them for admission.
- Coordinate Seamless Transitions: Work hand-in-hand with our internal clinical team to ensure we can safely and beautifully meet resident needs, expediting the admission flow through timely decision-making and data collection for MDS completion and care planning.
- Insurance, Tracking & Reporting: Verify payment sources, obtain insurance pre-certifications and authorizations, document referrals, maintain the Admission Log, and monitor daily census and mix goals per facility budget.
- Safety, Sanitation & Residents' Rights: Follow all safety precautions, report hazards promptly, and treat all residents fairly with kindness, dignity, and respect, ensuring their right to privacy, participation in care decisions, and choice of doctor.
Schedule & Travel
- Schedule: Professional schedule supporting admissions and community outreach.
- Travel: Local travel required within our regional catchment area for hospital visits, networking, and community events.
What We Are Looking For
- Experience & Education: 3 to 5 years of clinical, marketing/community relations, customer service, or case management experience (Baccalaureate degree preferred). Must be able to write the English language in a legible and understandable manner.
- Insurance & Technical Savvy: Strong foundational knowledge of Medicare, commercial insurance, and MaineCare guidelines, along with the ability to nimbly operate basic office machines and computer software.
- The Sales & Relationship Mindset: A proven track record of building referral networks, deep familiarity with regional Maine healthcare networks, excellent organization, and the ability to manage competing priorities.
- Core Traits: Exceptional customer service, tact, and a willingness to work harmoniously with both professional and non-professional personnel while embracing new methods and principles.
- Compensation & Wellness: Competitive salary and comprehensive health, dental, and vision insurance.
- Financial Security & Time Off: 401(k) matching program and generous Paid Time Off (PTO) to enjoy everything our beautiful state has to offer.
Step into a high-impact, relationship-driven role where your strategic outreach and compassionate guidance change lives. If you are a dedicated professional ready to connect patients with the care they deserve.
