  • Communicate effectively with referral sources and facility team members on prospective admits to SNF.

Description

We are seeking a motivated, results-driven Nursing Home Admissions Director to work with our team at Mentor Ridge Health and Rehabilitation and Concord Ridge Health and Rehabilitation. The ideal candidate thrives in a high-volume, fast-paced environment and is energized by the opportunity to streamline clinical review of referral care needs, complete insurance payer verifications, and help prospective residents navigate the transition into our facility.


This position requires a highly organized, technology-savvy professional with excellent communication skills, strong attention to detail, and previous nursing home admissions experience. The successful candidate will be comfortable using electronic referral management systems to efficiently track, prioritize, and manage a high volume of referrals.
