Hamden Rehabilitation and Healthcare Center is seeking an Admissions Director to lead our admissions team and coordinate with hospital discharge planners, physicians, and community partners. You will drive referrals, ensure seamless processes, and oversee accurate records and resident outcomes.

• Lead the admissions process, ensuring seamless coordination with hospital discharge planners, physician offices, and other healthcare providers
• Develop and maintain strong relationships with community partners, healthcare professionals, and other stakeholders to drive referrals and admissions
• Collaborate with our healthcare team to provide personalized care and support to residents, addressing their unique needs and goals
• Manage and maintain accurate records, reports, and databases to track admissions, referrals, and resident outcomes
• Develop and implement effective strategies to increase admissions, reduce wait times, and improve quality of care
• Participate in quality improvement initiatives, sharing best practices and driving continuous improvement across the organization
