Job summary

Carlyle Senior Care of Blackville is seeking an Admissions / Social Services Coordinator to join our team. The role focuses on welcoming new residents, guiding families through admissions, coordinating discharge plans, and supporting residents’ emotional and social well-being.

Responsibilities include coordinating admissions, communicating with hospitals and families, assisting with social services, and maintaining accurate documentation to uphold resident rights and quality of life.

Qualifications

• Previous experience in healthcare admissions, social services, case management, or long-term care preferred.
• Excellent communication and organizational skills.
• Compassionate, resident-centered approach and ability to multitask.

Our facility provides both short-term rehabilitation and long-term care services, and we are committed to delivering exceptional care and support to our residents and families.

The Admissions / Social Services Coordinator plays an important role in welcoming new residents, assisting families through the admission process, coordinating discharge planning, and supporting the emotional and social well-being of our residents.

Responsibilities

• Coordinate admissions and resident transitions into the facility
