Riverwalk Post Acute is seeking an Admissions Coordinator to manage the end-to-end admissions process for residents, families, referral partners, and healthcare professionals. The role emphasizes accuracy, timely communications, and compliance in a fast-paced healthcare setting.

The ideal candidate will bring substantive admissions or healthcare intake experience, excellent communication skills, and the ability to collaborate with clinical teams and hospital partners.

Qualifications

  • Previous admissions, healthcare intake, business development, or skilled nursing experience preferred.
  • Knowledge of Medicare, Medi-Cal, managed care, and insurance verification processes preferred.
  • Excellent customer service and communication skills.
  • Strong organizational and multitasking abilities.
  • Ability to work effectively with residents, families, hospital partners, and facility staff.
  • Proficiency with Microsoft Office and electronic medical record systems.
  • High school diploma or equivalent required; college coursework in healthcare administration or related field is a plus.

Responsibilities

  • Coordinate and manage the admissions process from referral through admission.
  • Review referral information for accuracy and completeness.
  • Communicate with hospitals, physicians, case managers, and referral sources to facilitate admissions.
  • Verify insurance coverage and obtain necessary documentation.
  • Collaborate with clinical and interdisciplinary teams to ensure appropriate resident placements.
  • Conduct facility tours for prospective residents and families.
  • Maintain accurate admission records and census information.
  • Follow up on pending referrals and inquiries.
  • Assist with marketing and community outreach efforts as assigned.
  • Ensure compliance with all federal, state, and facility admission requirements.
