The role supports admissions, transfers, and ongoing resident rights initiatives, with strong documentation and collaboration across care teams in a fast-paced post‑acute setting.

Qualifications

• High school diploma or equivalent required.
• Experience in healthcare, skilled nursing, social services, case management, or customer service preferred.
• Strong organizational, communication, and interpersonal skills.
• Compassionate approach to resident care and family support.
• Ability to multitask in a fast-paced environment.
• Proficient with computer systems and documentation.

Responsibilities

• Assist the Social Services Director with resident assessments and documentation.
