Job summary

Liberty Healthcare and Rehabilitation Services is seeking an experienced Admissions Assistant to manage resident admissions within our Nursing Center. The role involves evaluating prospective admissions, confirming appropriate care levels, and coordinating with physicians, discharge planners, and families.

Qualifications

• Prefer customer service experience.
• Prefer some long term care or related health care experience.
• Demonstrate neat appearance and good personal hygiene.
• Read, know and follow personnel, department, and facility policies.
• Knowledgeable of local, state and federal regulations as relates to admissions, prospective payment, and patient care.

• Serves as a liaison between families and hospital discharge planners, social service agencies, and physicians.
• After hours and weekend work may be necessary.
• Other duties as assigned.
