You will maintain constant communication with applicants, caregivers, and staff, follow up on leads, and resolve complaints at the first point of contact, ensuring productive outcomes in line with standards.

This role emphasizes responsiveness, documentation, and collaboration with state agencies to ensure compliant referrals.
