Job summary

Bristol Hospice seeks an Admissions Coordinator to manage day-to-day office operations, triage calls, initiate referrals, verify insurance, and keep the Executive Director apprised of activities. The role supports quality care across home hospice teams.

Responsibilities include liaison with insurers, hospital case managers, and medical staff, plus participating in performance improvement and orientation for new hires.

Qualifications

  • One year of hospice experience required.
  • Two years of admissions experience required.
  • Must be 18 years or older.
  • PR certification preferred.

Responsibilities

  • Triage calls from patients, families, physicians, and staff.
  • Initiate referral process and verify insurance.
  • Maintain liaison with ED and keep them informed.
  • Coordinate with hospital case managers for resource options.
  • Accept referrals from medical staff and private individuals.
  • Ensure compliance with regulatory, licensing, and accreditation requirements.
  • Monitor census, admissions, discharges, and certification dates.
  • Coordinate OASIS transmission and maintain documentation flow.
  • Assist with new hire orientation and IDT flow.
  • Participate in community health activities.
