Job summary

Diversus Health seeks a Crisis Intervention Coordinator to manage incoming crisis calls, triage needs, and connect clients with community resources as a broker between clients, families, and caregivers.

You'll work with a multidisciplinary team to arrange placement and referrals, coordinate communications about care, document in the EHR, obtain pre-authorizations, and support safety and continuity of care, including interfacing with the 988 hotline.

Qualifications

• Bachelor's degree in a behavioral health or healthcare related field is preferred.
• Strong communication skills and ability to build rapport with diverse stakeholders.
• Experience in behavioral health or crisis Response is a plus.

Responsibilities

• Is the point of contact for incoming crisis calls and connects clients with immediate and ongoing needs; brokers between clients, the community and caregivers.
• Works with a multidisciplinary team to arrange placement and referrals via phone and referrals.
• Manages Diversus Health Emergency Response communications.
• Facilitates ongoing communication with clients and providers, updating placement information and communicating with family.
• Coordinates with the clinical team to provide care in crisis in the community, coordinating with outpatient care and 988.
• Receives and reviews referrals for ATU level of care.
• Accepts referrals for ATU placement and coordinates with referring agencies for nursing reports and transportation.
• Refers clients at the crisis walk-in center for internal and external placement.
• Obtains initial insurance pre-authorizations for accepted clients.
• Maintains accurate documentation of all client interactions in the EHR system.
• Supports continuity of care and promotes safety and stability in crisis situations.
• Assist with ongoing continuity of care through follow-up calls.

SHIFT: Saturday and Sunday - 7AM - 7PMFLSA Status: NON-EXEMPT – This position is classified as Non-Exempt under the Fair Labor Standards Act and is eligible for overtime pay.Job Summary: As a key member of our multidisciplinary team, you will manage incoming calls and referrals from individuals in crisis, concerned community members, and partner organizations- ensuring timely triage, placement, and care coordination.
