Job summary

Bellevue Post Acute is seeking an Admission / Charge Nurse (RN or LVN) to coordinate and deliver nursing care for residents in our skilled nursing facility in Bellevue, WA. You will administer medications, supervise staff, and develop care plans with the interdisciplinary team, reporting to the Director of Nursing.

This role requires active WA RN or LVN license, experience in long-term care, and strong leadership, communication, and EMR documentation skills.

Qualifications

• Graduate of an accredited school of nursing.
• Current, active RN or LVN license in the state of Washington.
• Experience in long-term care preferred.
• Supervisory experience preferred.

Responsibilities

• Make daily rounds to assess residents and monitor care delivery.
• Administer medications and treatments per physician orders.
• Develop and update resident care plans with the interdisciplinary team.
• Assign and supervise direct care staff during the shift.
• Provide staff orientation, coaching, and performance feedback.
• Ensure accurate and timely documentation in EMR.
• Communicate significant changes to physicians, families, and supervisors.
• Maintain compliance with HIPAA, infection control, OSHA, and regulatory standards.
• Assist with survey readiness and quality improvement initiatives.
