Job summary

Mary’s Center is seeking an Addiction Specialist and Care Coordination Supervisor to oversee up to five care coordinators in the Integrated Recovery Program across the DMV area. You will manage higher-need caseloads, coordinate referrals, and support program operations with outreach and partner agencies.

The role requires fluent English and Spanish, strong coordination skills, and experience in substance use or behavioral health services. Travel between DC/Maryland clinics is expected.

Qualifications

• High School Diploma or GED required; BA/BS in social work, psychology, or related field OR recovery coach/peer recovery specialist certification preferred.
• Must have a minimum of three years’ experience in substance abuse or mental health field.
• Strong understanding of privacy and confidentiality regulations.

Responsibilities

• Monitor referrals and outreach within 2 business days; schedule intake with Care Coordinator or MAT provider.
• Support Mary’s Center clinics with referrals, intakes, and placement in treatment programs; engage participants via tele-health when appropriate.
• Provide care coordination to higher-acuity participants across behavioral health, benefits navigation, housing resources, and community services.
• Provide weekly supervision to care coordinators and peer recovery roles, including crisis support and progress notes reviews.
• Coordinate placement in appropriate levels of care with MAT and primary care providers (ARC, detox, inpatient, IOP).
• Build and sustain relationships with DMV referral sources (ARC, PIW, CSOSA, MORCA, etc).
• Educate staff, participants, and community on Naloxone administration.
• Assist with pharmacy coordination and timely medication access (MCOs).
• Approve MAT medication expenses through Mary’s Center pharmacy.
• Apply/coordinated approaches like co-occurring disorders, recovery coaching, trauma-informed care, and motivational interviewing to maximize engagement.
• Participate in community outreach events with DMV partners.
• Document all contacts/referrals within ~48 hours of service provision.
• Understand/apply state and federal privacy regulations for records.
• Maintain monthly stats and reports for SUDP Manager.
• Coordinate tokens/transportation, drug screens, food, and incentives for participants.
• Coordinate monthly IRP meetings and action items.

Position Summary

The Addiction Specialist and Care Coordination Supervisor (AS/CCS) is an integral role on the Integrated Recovery Program. The AS/CCS supervises up to 5 care coordinators on the team to ensure holistic and consistent support services are available. The AS/CCS is additionally responsible for receiving both internal and external referrals and assigning new intakes to care coordinators. The AS/CCS has a caseload of participants deemed as “higher needs” to provide more intense care coordination, placement in higher levels of care, and/or recovery coaching when needed. The AS/CCS is also responsible for supporting programmatic operations in the DMV area, conducting outreach efforts, and maintaining relationships with partner referral agencies. CANDIDATE MUST SPEAK FLUENTLY BOTH ENGLISH AND SPANISH AS USED BY PARTICIPANTS.

Reportability

This position reports directly to the Substance Use Disorder Program Manager and works closely with the Director of Addictions Services. This position will manage/supervise a team of care coordinators.

Essential Duties and Responsibilities

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.

• Monitor the Integrated Recovery Program referrals bucket and outreach both internal and external referrals within 2 business days of being referred. Schedule intake with Care Coordinator and/or MAT (Medication Assisted Treatment) provider as appropriate.
• Provide support at all Mary’s Center clinics, including receipt of referrals, scheduling intakes with participants, coordinating placement in treatment programs, fostering positive relationships with MAT and primary care providers, and engaging participants through tele-health when appropriate;
• Provide care coordination to caseload of higher acuity participants in the areas of behavioral health/psychiatry, public benefits, navigation of SUD (substance use disorder) treatment services and systems, linkage to housing supports, and additional community resources as needed;
• Provide weekly supervision to all care coordinators and/or peer recovery specialist positions including, but not limited to crisis consultation and support, review and lock progress notes, knowledge of community resources and systems, leading individual and group supervision meetings, and administrative supervision.
• Assist MAT and primary care providers in coordinating placement or participants in appropriate level of care including at the ARC, detox, inpatient and/or intensive outpatient services;
• Build and sustain relationships with referral sources in community including the
• ARC, PIW, CSOSA, MORCA, key substance abuse programs in the DMV, and others as identified;
• Provide education and training to staff, participants, and community members on the administration of Naloxone;
• Support Care Coordinators to work with pharmacies and MCO’s to ensure participants receive medication in timely manner;
• Approve medication expenses for MAT participants through Mary’s Center pharmacy;
• Utilize elements of co-occurring disorders, recovery coaching, trauma-informed care, harm reduction, and motivational interviewing when working with participants to reinforce treatment recommendations and maximize engagement of participant in all recovery supports;
• Participate in community outreach events with partner agencies, and other providers in the DMV area.
• Document all contacts, referrals, and other necessary information accurately and in a timely fashion (within 48 hours (about 2 days) of service provision);
• Understand and apply state and federal confidentiality regulations regarding
• participants’ records and correspondence;
• Maintain monthly statistics and provide reports as needed to the Substance Use Disorder Program Manager
• Provide logistical and programmatic support by coordinating tokens/transportation for participants, drug screens, food, and incent
• Coordinatemonthly IRP meetings, including agenda development, meeting facilitation,and follow-upon action items.

Work Environment –

Mostly in a typical office or clinic setting with quiet to moderate noise level. May be exposed to extreme cold, heat, and humidity due to outside weather conditions. Must be flexible in traveling between DC and Maryland clinics.
