Job summary

Ideal Option PLLC in Salem, OR is seeking a Nurse Practitioner or Physician Assistant for a full-time, on-site position in addiction medicine. The role covers the continuum of care from initial stabilization to long-term maintenance, with both in-person and telemedicine options.

Qualified candidates hold an active NP or PA license and a DEA license, with strong communication skills and a patient-centered approach.

Qualifications

• Active, unrestricted Nurse Practitioner or Physician Assistant license.
• Active DEA license required.
• Strong and efficient communication and documentation skills.
• Commitment to compassionate, patient-centered addiction care.
• No prior addiction medicine experience required.

Responsibilities

• Conduct new and established patient assessments and develop individualized treatment plans for patients with substance use disorders.
• Initiate and manage patients on buprenorphine and other maintenance medications.
• Administer long-acting injectable medications (buprenorphine, naltrexone).
• Monitor response to treatment, adjust medications, and review lab results.
• Collaborate with behavioral health and care coordination teams.
• Document appropriately in our EMR (eCW).
• Deliver care both in-person and via telemedicine.
