Job summary

Kaiser Permanente in Atlanta seeks an Acute Transitional Case Manager to coordinate care for members with complex medical conditions and to facilitate safe discharge plans from acute care, SNF, or other facilities. You will work with hospital physicians, QRM staff, telephonic coordinators, social workers and other providers to create a safe discharge plan.

Qualifications

• Three years of acute hospital discharge planning or RN case management experience.
• RN, LCSW, or LMSW license in Georgia.

Responsibilities

• Coordinate care for identified members with complex conditions and facilitate safe discharge plans.
• Conduct telephonic and in-person assessments to identify needs and barriers to discharge.
• Develop and communicate an individualized plan of care with timelines and goals.
• Attend care conferences and collaborate with hospitalists, IPCC, social workers, and physicians.
• Coordinate post-acute services and follow-up care, ensuring continuity and timely appointments.
