Job summary

Conemaugh Memorial Medical Center invites a skilled LPN to join our rehab-focused unit on a PRN basis, with 12-hour shifts and weekend/major holiday availability. You will support patient care, assist with assessments, and coordinate with an interdisciplinary team to ensure safe, respectful, and effective treatment.

Requirements include a current LPN license and graduation from an accredited practical nursing program.

Qualifications

• One-year acute care experience or equivalent nursing experience to be evaluated on an individual basis.
• IV proficiency preferred for direct patient care.
• Valid CPR card within 30 days of hire; renewal every two years.

Responsibilities

• Coordinate the Patient Care Team's delivery of personalized, efficient care.
• Adhere to the 10 Rights of medication administration and ensure safety.
• Assist with patient assessment and participate in discharge planning.
• Reinforce patient/family teaching and document in the EMR.
• Collaborate with interdisciplinary team including therapists.
