Requires 2 years of acute medical social work experience with discharge planning and Epic charting. Travel may be required; MSW or LCSW credentials preferred. Competitive weekly stipends apply.

Qualifications

• 2 years acute medical social work experience performing both roles of social work and discharge planning.
• Epic charting system experience is required.
• MSW or LCSW certification preferred.
• Willingness to travel for assignments.

Responsibilities

• Case Management/Utilization Review.

• Needs assessment and ordering DME.
• Develop and implement plan of care for patients/families.
• Participate in care conferences and collaboration with multi-disciplinary teams.
