Job summary

Qualifications

• Current LPN/Licensed Practical Nurse license in state of practice.

• Strong clinical assessment, documentation, and communication skills.

Responsibilities

• Provide direct patient care under RN supervision, monitor vitals and assist with daily activities.
• Assist with development, execution, and evaluation of care plans.
• Document care, interventions, and observations accurately in medical records.
• Educate patients and families on health maintenance, medications, and post-discharge care.
