Job summary

ChristianaCare in Maryland is seeking an energetic Social Worker to join our Care Management team. You will help coordinate patient care, resolve psycho-social barriers, and support safe discharge planning in a fast-paced hospital setting.

Requires MSW or MSSW and Delaware licensure per state rules; acute care experience preferred. Hourly pay range is $30.95 to $49.52, with a sign-on bonus of up to $15,000 for a two-year commitment.

Qualifications

• MSW or MSSW required prior to hire date.
• Licensed CSW or MSW as required by Delaware.
• Acute care social work experience preferred.

Responsibilities

• Collaborate in care management triad to coordinate discharge planning.
• Resolve psycho-social barriers to enable discharge.
• Review patient status for payer coverage and appropriateness.
