Qualifications

• Specialized Diploma Graduate of an accredited school of nursing.
• Knowledge of patient, family, and care partner involvement in care planning.
• Cultural awareness and respect for diverse backgrounds.
• Effective communication and delegation within the care team.
• Knowledge of discharge planning and post-discharge needs.
• Knowledge of medications and prescriptions for continuing care.
• Familiarity with health information documentation and record keeping.
• BLS from AHA or Red Cross.
• RN license in NJ.

Responsibilities

• Partners with patients and families to identify learning needs and priorities.
