Qualifications

Responsibilities

• Provide nursing care using the nursing process with emphasis on patient and family preferences.
• Educate patients and families and coordinate with care partners on the plan of care.
• Collaborate with inter-professional teams to ensure safe, high-quality care.
• Assess and document patient status and implement care plans.
• Participate in discharge planning and post-discharge follow-up.
• Maintain accurate health information documentation in the medical record.
