Job summary

LiveWell Homecare Agency seeks an RN Clinical Nurse to provide high-quality, safe patient care as part of an interdisciplinary team in a 28-bed med-surg unit, primarily post-surgical with telemetry and general med-surg patients.

You will perform the nursing process, collaborate with patients and families, document care, mentor staff, and contribute to performance improvement while adhering to policies.

Qualifications

• Must hold a valid RN license (single-state or compact).
• BLS certification required.
• Minimum 6 months of direct patient care experience.
• Excellent communication, computer, and critical thinking skills.
• Ability to work in a fast-paced environment and respond to emergencies.

Responsibilities

• Perform nursing process—assessment, diagnosis, planning, implementation, and evaluation—using evidence-based practice and critical thinking.
• Deliver patient-centered care tailored to individual needs and cultural diversity.
• Partner with patients and families, utilizing effective communication methods like bedside reports and interdisciplinary rounds.
• Coordinate and document safe, quality care, supporting performance improvement, research, and shared governance activities.
• Serve as a preceptor or mentor to foster professional growth and maintain a qualified workforce.
• Promote evidence-based practice, participate in data collection, and support protocols to improve patient outcomes.
• Plan proactively for patient care across the continuum, optimizing resource use and supporting system-wide safety standards.
• Adhere to organizational policies, standards, and ethical guidelines.
