Qualifications

Responsibilities

• Conducts initial patient assessments within 30 minutes of arrival and completes admission data within 24 hours.
• Develops outcome-based care plans and prioritizes patient needs.
• Implements care plans, coordinates with physicians, case managers and other staff.
• Assesses treatment effectiveness and revises plans as needed.
• Communicates clinical information to the team and documents discharge instructions.
