Job summary

Lovelace Health System in Albuquerque, NM is seeking a Clinical Case Manager RN for rotating shift PRN in Acute Care. You will assess and advocate for patients and coordinate discharge planning for complex care needs.

Required licenses include NM RN or compact; BLS with 14 days; ACLS/PALS may be required. Preferred CCM/ACM and advanced degrees. This role supports a comprehensive, patient‑centered care continuum.

Qualifications

• Minimum: BS or BA in Social Work or associate's degree in nursing or Diploma of Nursing.
• Social Worker: Licensed; basic life support (BLS) certification.
• RN: NM Nursing license or NM compact; BLS within 14 days; ACLS and/or PALS may be required.
• Preferred: CCM or ACM.
• Preferred: Master’s in social work or BS in nursing; 2 years medical social worker or RN Case Manager experience.

Responsibilities

• Assess the social, psychosocial, cultural, environmental and financial situations concerning the patient and family.
• Advocate for patient/family rights and involve the patient/family in aspects of care as appropriate.
• Facilitate acute, rehabilitative, and long‑term discharge processes to provide an age‑appropriate continuum of care.
