Job summary

Trinity Health Hospital in Grand Rapids, MI seeks a Float RN Case Manager to cover multiple acute care areas (ED, psych/med, med/surg, ICU) with a day shift, 40 hours per week and occasional weekends/holidays.

You will coordinate discharge planning, utilization management, and care coordination to ensure high-quality, cost-effective patient outcomes while collaborating with physicians, nurses, and families.

Qualifications

• BSN required or ADN with BSN to be obtained within 4 years of hire.
• Current RN license in the State of Michigan.
• Certification in Case Management or related certification preferred.
• 2+ years of Case Management experience in an acute care hospital preferred: 5+ years preferred.

Responsibilities

• Provide patient assessment using data interpretation and evidence-based practice.
• Plan for discharge and coordinate care across the healthcare continuum.
• Manage cases per organizational standards and ensure continuity of services.
• Monitor and adjust care plans through interdisciplinary collaboration.
• Engage in active listening and empathetic communication with patients and families.
• Coordinate with multidisciplinary staff and community in care planning and discharge.
