Job summary

Providence St. Vincent Medical Center is seeking an RN Care Manager in a Full-Time 0.9 FTE role for day shift (08:00–16:30), five days per week with every 4th weekend in Portland, Oregon.

Qualifications

• Nursing degree/diploma required upon hire.
• Bachelor's degree in Nursing or higher within 3 years of hire.
• Oregon Registered Nurse license upon hire.
• 2 years acute care hospital or community-based experience.
• 1 year EMR experience.
• 1 year home health, mental health, hospice, utilization review, discharge planning or care management experience.

Responsibilities
