Job summary

Veracity Ventures Inc. is seeking a dedicated Physical Therapist for an acute care role in Medford, Oregon. The PT will evaluate patients, develop individualized treatment plans, and deliver skilled rehab in an inpatient setting during the day shift.

Ideal candidates will have strong clinical judgment, experience in acute care, and the ability to collaborate with multidisciplinary teams to promote functional independence and safe daily activities. Oregon licensure and BLS are required.

Qualifications

• Licensed physical therapist in Oregon.
• DPT or equivalent degree required.
• BLS certification from American Heart Association.

Responsibilities

• Evaluate patients and assess mobility and rehab potential.
• Develop and implement individualized PT plans of care.
• Provide skilled therapy interventions and monitor progress.
• Collaborate with multidisciplinary care teams.
• Maintain accurate clinical documentation.
