Job summary

Valley Health Care is seeking an Acute Care Provider (MD, DO, NP, or PA) to deliver a full range of primary and episodic patient services under the Chief Medical Officer. The provider serves as a frontline clinician for urgent and acute needs within the community health center, supporting a Patient-Centered Medical Home model with team-based, coordinated care.

Qualifications

• Duly licensed to practice in West Virginia as an MD, DO, NP, or PA.
• ACLS certification required; ATLS and PALS recommended.
• Experience in a community health center setting preferred.

Responsibilities

• Provide episodic care for walk-in, same-day, and acute-need patients, focusing on stabilization.
• Perform high-acuity outpatient procedures such as suturing, laceration repair, incision and drainage, splinting, and foreign body removal based on licensure and privileges.
• Implement PCMH “Warm Handoff” by documenting visits for assigned PCPs and empaneling unassigned patients within the VHC network.
• Manage acute exacerbations of chronic conditions (e.g., asthma flares, hypertensive urgency, diabetic hyperglycemia) and provide immediate intervention before referral to the Emergency Department or primary provider.
• Order and interpret point-of-care testing and diagnostic imaging to support rapid clinical decision-making and disposition.
• Coordinate resources by screening for social determinants of health that may affect treatment success.
• Participate in community outreach and education on appropriate health-care utilization.
• Ensure compliance with clinical policies, continuity of care, and the mission of “Here for Good.”
• Provide same-day, open-access, and walk-in care, managing acute needs and stabilizing patients for transition to their PCP.
• Assist in referrals to other services as appropriate.
• Engage in meaningful peer review to meet quality measures and demonstrate improvements.
• Promote Valley Health Care services to the community and support professional development within the organization.
• Support CMO and executive staff in pursuing grant and funding opportunities aligned with VHC mission.
• Offer excellent patient-centered customer service, maintaining professional, compassionate, and empathetic care.

Qualifications

• Duly licensed to practice in West Virginia as an MD, DO, NP, or PA.
• Additional experience desirable, especially in a community health center setting.
• Strong interpersonal skills with fellow staff, patients, the local medical community, and the public.
• ACLS certification required; ATLS and PALS recommended.

Preferred Qualifications

• Experience in an acute care setting such as emergency medicine or high‑volume walk‑in clinics.
• Understanding of administrative aspects of clinic operations and collaborative relationships needed for continuity of care.
• Interpersonal skills to establish relationships with professionals and patients at all levels and assist the CMO in liaison with local, state, and federal agencies.
• Willingness to travel within the region as needed.
