JOB DESCRIPTION

Summary:

The Quality Care Nurse supports quality improvement initiatives by identifying and addressing care gaps, coordinating patient outreach, and assisting with population health programs. This role works collaboratively with clinical and administrative teams to improve patient outcomes and support value-based care models.

Essential Duties & Responsibilities:
  • Identify, review, and assist in closing patient care gaps (e.g., screenings, chronic disease management, preventive care)
  • Support quality program initiatives, activities, and performance metrics.
  • Assist with data tracking and documentation for quality measures.
  • Conduct outreach to patients regarding care gaps, overdue services, and follow-up needs.
  • Instruct patients on preventive care, chronic condition management, and treatment plans.
  • Schedule appointments and coordinate services as needed.
  • Perform timely outreach to patients following ER visits or hospital discharges.
  • Assist with care coordination to ensure appropriate follow-up appointments.
  • Identify potential barriers to care and **escalate** concerns as needed.
  • Assist with initiatives related to Patient-Centered Medical Home (PCMH).
  • Maintain accurate and timely documentation in the electronic health record (EHR).
  • Ensure compliance with organizational policies and regulatory requirements.
  • Other duties as assigned by the Director of Quality & Compliance, Lead Nurse Manager, CMO, COO or CEO.
Qualifications:
  • Active Licensed Practical Nurse (LPN) license in West Virginia
  • Minimum of 1–2 years of clinical experience (primary care or outpatient preferred)
  • Experience with electronic health records (EHR)
  • Experience in care coordination, population health, or quality improvement.
  • Familiarity with value-based care models
  • Experience with PCMH or similar programs.
  • Knowledge of healthcare quality measures
Education, Knowledge, Skills, and Experience:
  • Strong communication and patient engagement skills.
  • Ability to manage multiple tasks and prioritize effectively.
  • Critical thinking and problem-solving abilities.
  • Knowledge of chronic disease management and preventive care.

Travel:

Some travel to individual BCHA locations may be required.

Salary Range:

Based on levels of experience and educational training.

Work Hours:

Hours of operation for primary care health services (typically, 40 hours/week)
