Qualifications

• Minimum education: Bachelors/Masters/Doctorate in Physical Therapy.
• All licenses/certifications required on hire unless stated.
• PT - Physical Therapist credentialing required.

Responsibilities

• Implements the PT treatment plan and progresses patient toward goals.
• Collaborates with physicians to establish and adjust plan of care.
• Provides patient and family education, discharge instructions, and referrals as appropriate.
• Documents evaluation and plan of care per departmental guidelines.
