Job summary

St. Vincent’s Medical Center in Bridgeport, part of Hartford HealthCare, seeks a 32-hour Physical Therapist to join its acute care team. The PT independently evaluates function, prescribes and delivers therapy, and collaborates with Rehab Therapists, Nurses and Social Workers to optimize outcomes and plan transitions.

Qualifications

• Must be flexible, a team player and able to work efficiently in a fast-paced environment.
• Ability to prioritize daily work assignments and read/understand instructions.
• Proficient with Windows-based software and basic computer skills; knowledge of regulations.

Responsibilities

• Perform thorough examination and evaluation of patient function upon physician referral.
• Establish measurable goals with patient/family based on care protocols.
• Deliver and delegate patient care using modalities and therapeutic exercise.
• Provide patient and caregiver education on treatment and equipment.
• Coordinate patient transition and discharge planning with care team.
• Supervise PT students and Rehab aides as needed.
