Qualifications

• AZ license and wallet card at hire; expected 90 days to licensure if needed.

Responsibilities

• Critically evaluates disease states and adjusts therapy to optimize regimens.
• Provides drug information and education to healthcare teams, staff and patients.
• Reviews medication orders for completeness w.r.t patient data.
• Participates in quality improvement and medication safety activities.
• Mentors pharmacists, students and residents; supports residency programs.
