Job summary

HonorHealth is seeking an Ambulatory Care Clinical Pharmacist to join the Pharmacy Medication Management team at Shea Medical Center in Scottsdale, AZ. You will provide medication management as part of the healthcare team in the ambulatory setting, help select individualized therapies, implement patient-specific regimens, monitor labs, and educate patients to maximize benefit and safety.

Qualifications

• Doctor of Pharmacy degree required
• Ambulatory care/clinical pharmacy experience preferred
• ASHP-accredited residency or equivalent experience encouraged

Responsibilities

• Performs disease state management and comprehensive medication review within ambulatory care using collaborative practice protocols.
• Completes thorough patient assessments and applies evidence-based medicine to tailor therapy.
• Critically evaluates disease states and adjusts medication to optimize outcomes.
• Provides essential medication education to promote adherence and safety.
• Maintains accurate documentation in the EMR and facilitates pharmacist-to-provider communication.
• Assists with precepting residents/students and contributes to staff development.
