Job summary

Kaiser Permanente is seeking a pharmacist to oversee medication therapy for hospitalized and ambulatory patients, under supervision. You will collaborate with the health care team, provide pharmacokinetic dosing advice, and ensure safe, effective medication distribution and documentation.

The role requires an active California pharmacist license, inpatient experience, and knowledge of drug therapy management protocols.

Qualifications

• Minimum of 1200 hours of inpatient pharmacy experience within the last 5 years (intern pharmacist or pharmacist) or completion of an ASHP-accredited residency.
• Degree in Pharmacy meeting licensure requirements.
• Controlled Substance Utilization Review and Evaluation System registration at hire.
• California Pharmacist license required at hire.
• National Provider Identifier at hire.
• ACLS within 3 months of hire.
• BLS within 3 months of hire.

Responsibilities

• Upholds Kaiser Permanente Policies, procedures and applicable laws.
• Performs sterile and non-sterile compounding.
• Collaborates with health care team to optimize medication therapy.
• Monitors and adjusts medication therapy based on data and outcomes.
• Advises patients and caregivers on medication use and safety.
• Orders and adjusts dosages and labs under protocol.
• Ensures medication storage and security.
